H1N1 Flu Vaccine

Parents who would like their child to receive the H1N1 vaccine can continue to return consent forms until the day of the school clinic.
Following the clinic, a vaccination card will be sent home with the child. We will not force a child to receive the vaccine. If you would like to be present, please be available at clinic start time until your child is called. If a child is not cooperative on clinic day, a vaccine refusal letter will go home notifying parents the vaccine was not given. Parents can then go to one of the Metro Public Health Department sites for the free vaccine. Parents who have turned in a consent form but who have since had their child vaccinated outside school should notify the school immediately to prevent their child from receiving another vaccination. If the child has already received one dose of the vaccine, he or she can receive the second dose at school but again parents need to contact the school immediately. Along with a consent form, the school must be provided with the date the first dose was given, the form of vaccine (mist or shot) and a request that the second vaccination be given at school.
